Terms and conditions for FIT4MOM Chicago Silver Membership

FIT4MOM MONTHLY MEMBERSHIP AGREEMENT
This contract is made between the client and Fit4Mom Chicago (Wilder Fitness LLC) and is a month to month agreement and except as provided in the initial 10 day cancellation policy below, may be terminated by either party with the proper advance notice. This membership allows you to attend 5 drop-in classes a month.


AUTHORIZATION FOR PAYMENT
I authorize Fit4Mom Chicago to initiate a charge to the account entered into Pike13 for my dues. This authorization is to remain effective until I have cancelled my membership through the Fit4Mom Chicago administrator.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with Fit4Mom to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started for the first month and then on the 1st of the month going forward. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.

3 MONTH COMMITMENT:

I understand that I am agreeing to a 3 month membership commitment and must make 3 payments (initial partial month payments count towards commitment) before ending my membership. If I wish to end my membership at the end of 3 months, I must notify FIT4MOM Chicago at least 15 days prior to the first of the month using the form found here: https://goo.gl/forms/ORaIqDhzaujb5lsH3


CANCELLATION POLICY
To cancel this membership, the member must contact the Fit4Mom Chicago administrator 15 days prior to their next payment due date to request a cancellation. A cancellation form can be found at https://goo.gl/forms/ORaIqDhzaujb5lsH3. No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the Chicago franchise. Once a membership is cancelled, if the member wants to re-enroll they will be charged half the normal registration fee.

HOLD POLICY
Memberships may be held a maximum of 4 months per hold period. Hold starts will coincide with the next payment date of the first of the month and notice must be given 15 days in advance to hold. When a hold period is ended, the monthly auto payments will resume on the return date. There will be a $20 processing fee charged to your credit card on file for the hold period. This fee will be credited to your account upon your return. The only exception to this hold policy are those who become injured and expectant moms who deliver and plan to return to class after recovery. MEMBERSHIP DOWNGRADE POLICY

Memberships may be changed with 15 days notice from the billing date. There is no fee to change memberships and no new 3 payment commitment. To change memberships, please fill out the form found here: https://forms.gle/w2RJsw5g7wCwLm448

INITIAL 10-DAY CANCELLATION POLICY
You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement written notice must be received by Fit4Mom within the initial ten days. You are responsible for paying for the classes already attended.

CLASS REGISTRATION AND CANCELLATION POLICY

Please register if you plan to attend class and cancel your registration up to 1 hour before class. Cancellations less than 1 hour before class and no shows will be charged a late cancel fee of up to $10.
