Terms and conditions for Basic 3 Month Autopay Membership

Welcome to FIT4MOM Chicago! Below you will find our policies and information on how to contact us with any questions or concerns you may have regarding your membership or class pass.


Basic 3 Month Autopay Membership AGREEMENT:

I understand that I am agreeing to a 3 month membership commitment and must make 3 payments (initial partial month payments count towards commitment) before ending my membership. Payments will be automatically withdrawn on the first of each month via the payment method entered into my client account. If I wish to end my membership at the end of 3 months, I must notify FIT4MOM Chicago at least 15 days prior to the first of the month. An early cancellation is subject to a $99 fee. This membership allows attendance of 4 drop-in classes a month.


AUTHORIZATION FOR PAYMENT:

I authorize FIt4MOM Chicago to initiate a charge to the account entered into Pike13 for my dues. This authorization is to remain effective until I have cancelled my membership through the FIT4MOM Chicago administrator.


PAYMENTS FOR CLASSES:

All monthly memberships must have a credit card on file. Payments must be received in full prior to beginning any class. Monthly payments for debit/credit cards will be charged automatically on the first of each month. If a Member should change debit/credit card companies, have an updated expiration date, or a new billing address, these changes must be submitted via client Pike13 accounts. Funds that are denied for any reason will be subject to a $15 re-processing fee. Please see below for our membership hold and cancellation policy.


CREDITS/REFUNDS:

There are no credits or refunds given for missed classes due to weather or personal circumstances for ANY of our fitness programs.


CLASS REGISTRATION AND CANCELLATION POLICY:

Please register if you plan to attend class and cancel your registration up to 1 hour before class. Cancellations less than 1 hour before class and no shows will be charged a late cancel fee of up to $10.


HOLD POLICY:

Members may place their membership on hold for reasons such as; pregnancy leave, long extended vacations, injuries, etc. All holds can be for a period of no less than 30 days and no longer than 4 months each year, not to exceed two different hold periods per calendar year. All Clients wishing to activate holds must notify FIT4MOM Chicago administrator at chicago@fit4mom.com and complete the Hold Request Form 15 days prior to the desired hold start date. Any Membership will not be put on hold until the Hold Request Form has been completed and received by FIT4MOM Chicago administrator. Any requests received after a Member’s billing date will be prorated accordingly- holds do not need to coincide with billing dates. There is a one time $20 charge to place a membership on hold. Once the end of the hold period has been reached, the Member’s original Membership contract and monthly auto payments will resume . If a Member wishes to cancel during a hold period, the Member will still be responsible for full payment of their existing contractual agreement- hold periods DO NOT count towards the Member's monthly contractual obligation.


CANCELLATION POLICY:

Should a member decide to no longer attend classes, the member is still required to fulfill the contractual agreement for their Membership Package. Once the contract is fulfilled, 15 days notice is still required in order to process any cancellations. To cancel a membership, the member must request a cancellation form from the FIT4MOM Chicago administrator. A Cancellation Form must be completed and received by FIT4MOM Chicago 15 days prior to their next payment due date. Without an email confirmation from a FIT4MOM Chicago administrator and completion of the online Cancellation Form, the cancellation does not exist. It is up to the Member to obtain this confirmation within 72 hours of the desired cancelation date. There are no exceptions and no refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the FIT4MOM Chicago franchise. Once a membership is cancelled, if the member wants to re-enroll they will be subject to the enrollment fee.


INITIAL 10-DAY CANCELLATION POLICY:

You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement written notice must be received by FIT4MOM Chicago within the initial ten days. You are responsible for paying for the classes already attended.

**INCLEMENT WEATHER: Although rare, FIT4MOM Chicago will cancel classes in cases of inclement weather. Class cancellations due to these conditions will be announced at least 1 hour prior to class start time. **PRE ENROLLMENT FOR CLASSES: We require ALL clients to pre enroll for their classes. This allows instructors to contact clients should there be any changes to meeting spots or cancellation of classes. Clients must pre enroll at least TWO HOURs before class start time. Classes may be canceled if there are no pre enrollments. Any class cancellations will be announced at least ONE HOUR prior to class start time. A late cancellation fee of $10 may apply if a client enrolls for a class and is a "no show" in the system.


STROLLER POLICY FOR STROLLER STRIDES/STROLLER BARRE/STRIDES360

Children must remain in their stroller throughout class for their safety and the overall safety of the class.


STAY INFORMED:

Our main method of communication with you regarding FIT4MOM Chicago programming is through e-mail. If you do not receive our monthly newsletter or calendars, please let us know so we can confirm your e-mail information. Many e-mail programs have spam filters. You may want to check your system and add FIT4MOM Chicago to your “acceptable” e-mail list so that our e-mails are not blocked. ****Please email chicago@fit4mom.com for all business matters, we cannot respond to business emails via social media platforms.


MANAGING YOUR MEMBER & CLASS PASS CLIENT ACCOUNTS:

All FIT4MOM Chicago Members and Class Pass clients may manage their personal accounts by visiting: https://chicago.fit4mom.com and selecting the “My Account” tab along the menu bar. This will direct all clients to our Pike13 client account management system. Clients may also access this site by visiting https://fit4mom-chicago.pike13.com/ directly. All FIT4MOM Chicago Members and Class Pass clients must have their Liability Release waiver signed online via their Pike13 account prior to participating in any of our programs.

By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice.
